My guess:
It means that element 0 is different in the two lists which means that
bidirectional encoding (and in particular direct encoding) will normally
be between two different reference pictures and as I recall it is more
efficient (in terms of coded bits) to select between L0[0]/L1[0] than it
is to select between L0[0]/L0[1].
